ÀÍÎÒÀÖ²ß:
Ìåòà. Îö³íèòè îñîáëèâîñò³ ðîçâèòêó àðòåð³àëüíî¿ ã³ïåðòåí糿 ó ëþäåé, ÿê³ ìàþòü ìåòàáîë³÷íèé ñèíäðîì òà îá´ðóíòóâàòè çàõîäè, ùîäî ïðîô³ëàêòèêè.
Îá’ºêò ³ ìåòîäè äîñë³äæåííÿ. Áóëè îáñòåæåí³ 92 äîðîñëèõ ïàö³ºíòà â³êîì â³ä 40 äî 49 ðîê³â, ñåðåä íèõ 73 æ³íêè ³ 19 ÷îëîâ³ê³â. Îïèòóâàííÿ ïðîâîäèëîñÿ ñåðåä ðåñïîíäåíò³â çà äîïîìîãîþ àíêåòóâàííÿ. Ô³êñóâàëèñÿ çàãàëüíîêë³í³÷í³ ïîêàçíèêè: âèçíà÷åííÿ çðîñòó, âàãè ç íàñòóïíèì ðîçðàõóíêîì ³íäåêñó ìàñè ò³ëà (êã/ì2), îá’ºìó òà볿, ñèñòîë³÷íîãî òà ä³àñòîë³÷íîãî àðòåð³àëüíîãî òèñêó. Ñåðåä ëàáîðàòîðíèõ ïîêàçíèê³â óâàãó ïðèâåðíóëè íàñòóïí³: ãëþêîçà, ³íñóë³í, ³íäåêñ Homa, ëåïòèí, êðåàòèí³í, öèñòàòèí-ñ, øâèäê³ñòü êëóáî÷êîâî¿ ô³ëüòðàö³¿ (ØÊÔ). Ñòàòèñòè÷íà îáðîáêà ðåçóëüòàò³â ïðîâîäèëàñÿ çà äîïîìîãîþ êîðåëÿö³éíîãî àíàë³çó, âèçíà÷åííÿ êðèòåð³¿â â³äì³ííîñòåé, ãðàô³÷íèìè ìåòîäàìè.
ϳä ÷àñ äîñë³äæåííÿ áóëè äîòðèìàí³ ïðèíöèïè á³îåòèêè, à òàêîæ ïîëîæåííÿ Íàêàçó ̳í³ñòåðñòâà îõîðîíè çäîðîâ’ÿ Óêðà¿íè «Ïðî çàòâåðäæåííÿ ïîðÿäêó ïðîâåäåííÿ êë³í³÷íèõ âèïðîáóâàíü» ³ Òèïîâîãî ïîëîæåííÿ ïðî êîì³ñ³þ ç ïèòàíü åòèêè ¹66 â³ä 13.02.2006. Óñ³ ïàö³ºíòè ï³äïèñàëè ³íôîðìîâàíó çãîäó íà ó÷àñòü â îáñòåæåí³.
Ðåçóëüòàòè äîñë³äæåííÿ òà ¿õ îáãîâîðåííÿ. Ñòâîðåíî ðåºñòð ðåñïîíäåíò³â – äîðîñëèõ ÷îëîâ³ê³â òà æ³íîê, ÿê³ îïèíèëèñÿ ï³ä âïëèâîì ìîäèô³êîâàííîãî ÷èííèêà – íàäì³ðíî¿ âàãè ÷è îæèð³ííÿ, ùî º òðèãåðîì ïîøèðåíîñò³ òàêèõ ñòàí³â, ÿê ³íñóë³íîðåçèñòåíòí³ñòü, ëåïòèíîðåçèñòåíòí³ñòü, àðòåð³àëüíà ã³ïåðòåíç³ÿ. Äîñë³äæåííÿì âñòàíîâëåíî, ùî 73% îáñòåæåíèõ ìàëè íàäì³ðíó âàãó ÷è ð³çí³ ñòóïåí³ îæèð³ííÿ. Òàêîæ áóëî ï³äòâåðäæåíî äîñòîâ³ðíèé çâ’ÿçîê ì³æ ê³ëüê³ñíèìè ïîêàçíèêàìè çà äîïîìîãîþ êîðåëÿö³éíîãî àíàë³çó ϳðñîíà (r) ì³æ êîëîì òà볿 ³ â³êîì (r=0,55, p<0,002),çðîñòîì (r=0,547, p<0,002), âàãîþ (r=0,547 p<0,002), ²ÌÒ (r=0,753, p<0,001), êðåàòèí³íîì (r=0,548, p<0,002). Öå ùå ðàç ï³äòâåðäæóº, ùî êîëî òà볿 çá³ëüøóºòüñÿ ç â³êîì, çðîñòîì, âàãîþ, ³íäåêñîì ìàñè ò³ëà, à çá³ëüøåííÿ êðåàòèí³íó çàëåæèòü â³ä êîëà òà볿 ³ çðîñòó (r=0,364, p<0,048). Çàâäÿêè äèñïåðñíîìó àíàë³çó áóëè âèÿâëåí³ äîñòîâ³ðí³ çâ’ÿçêè ì³æ ê³ëüê³ñíèìè òà ðàíãîâèìè ïîêàçíèêàìè: ³íäåêñîì Íîmà ³ ñïîðòîì (F=1,809, P=0,049), à òàêîæ ëåïòèíîì ³ ñïîðòîì (F=1,809, P=0,049), ëåïòèíîì ³ âàãîþ (F=0,644, P=0,035).
Âèñíîâêè. Ïîãëèáëåíå êîìïëåêñíå îáñòåæåííÿ ç îäíî÷àñíèì âèçíà÷åííÿì àíòðîïîìåòðè÷íèõ, ãåìîäèíàì³÷íèõ, ãîðìîíàëüíèõ ïîêàçíèê³â ñòàíó çäîðîâ’ÿ äîçâîëèëî îá´ðóíòóâàòè ïðîô³ëàêòè÷í³ çàõîäè ç ìåòàáîë³÷íîãî ñèíäðîìó.
Ïîêàçàíî, ùî 73,0% ðåñïîíäåíò³â ìàëè íàäì³ðíó âàãó òà îæèð³ííÿ, ùî ìîæíà ðîçö³íþâàòè ÿê ôàêòîð ðèçèêó ðîçâèòêó àðòåð³àëüíî¿ ã³ïåðòåí糿.
Âñòàíîâëåíî äîñòîâ³ðíèé çâ’ÿçîê ì³æ ê³ëüê³ñíèìè ïîêàçíèêàìè çà äîïîìîãîþ êîðåëÿö³éíîãî àíàë³çó ϳðñîíà (r) ì³æ êîëîì òà볿 ³ â³êîì, çðîñòîì, âàãîþ, ²ÌÒ, êðåàòèí³íîì.
Âèçíà÷åíî, ùî öèñòàòèí-ñ íåãàòèâíî êîðåëþº ç³ øâèäê³ñòþ êëóáî÷êîâî¿ ô³ëüòðàö³¿ ïðè â³äñóòíîñò³ çá³ëüøåííÿ êðåàòèí³íó.
Ïðîô³ëàêòè÷í³ ä³¿ ç ìåòîþ ïîïåðåäæåííÿ ðîçâèòêó ðèçèê³â óñêëàäíåíü àðòåð³àëüíî¿ ã³ïåðòåí糿 ïðîïîíóºòüñÿ âïðîâàäèòè ëþäÿì ç ìåòàáîë³÷íèì ñèíäðîìîì (ÿê ÷îëîâ³êàì, òàê ³ æ³íêàì).
